Financial Policy

Medicare
We are a participating provider with Medicare, which means we will file claims on your behalf.  Under the Medicare program, each patient is responsible for a 20% co-payment as well as a Medicare deductible.  As a courtesy, unless you request otherwise, we will also file your secondary insurance.|
 
Managed Care (HMO, PPO)
We participate with most major managed care plans.  If your visit or treatment needs to be pre-authorized, please make sure this is completed by your primary care physician or managed care plan prior to your appointment.  Otherwise, you may find yourself responsible for the entire cost of your visit or procedure.

Medicaid and Social Services
We participate in Florida Medicaid and Medipass.  However, eligibility and authorization must be documented in order for us to file your claims.

Other Plans
Our policy is to collect fees at the time of service unless other arrangements have been made.  For your convenience, we accept most major credit cards.
